Confidential Opportunity - Social Services Coordinator (Long-Term Care) We are seeking a compassionate and experienced Social Services Coordinator to join a well-established long-term care and rehabilitation facility. Position Overview The Social Services Coordinator is responsible for supporting residents and families through the admission process, care planning, discharge planning, and ongoing psychosocial needs. This role serves as an advocate for residents while collaborating with the interdisciplinary care team to ensure the highest quality of care.
