Case Manager RN Registered Nurse Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 8 hours Experience: 3 years of inpatient hospital or acute care experience required; first-time travelers accepted with relevant experience License: Active RN state license required Certifications: BLS preferred Must-Have: - Experience with EPIC charting preferred - Ability to float between facilities as needed - Experience in acute hospital settings Description: The position requires a Registered Nurse to provide case management services. This role involves coordinating medical home care, placement, and addressing social issues to ensure safe discharges. Daily responsibilities include handling approximately 15 cases daily. The ideal candidate will possess skills in care coordination, discharge planning, and conducting assessments. Knowledge of relevant guidelines and coding systems is essential. The role may require occasional weekend shifts based on staffing needs. Interdisciplinary support will be available, including administrative assistance. Familiarity with outpatient and post-acute resources is beneficial. Candidates must be comfortable with floating and be adaptable to various responsibilities within the role.
