What You Will Do The Care Specialist primarily works in patients' homes and communities (90%) and virtually (10%) to deliver chronic care management to high-complexity patients. Responsibilities include informing patients about services, enrolling them, conducting outreach via phone or in person, and supporting virtual visits to promote timely, accessible, and coordinated care to improve health outcomes and reduce costs. Key Responsibilities Conduct outreach via phone, in-person, mailings, including cold calling and door knocks. Enroll patients and gather key data during enrollment through ongoing interactions. Meet patients at home or in the community for needs assessment and health goal setting. Coordinate care, facilitate provider visits, and ensure follow-ups. Support patients in meeting healthcare goals related to quality measures. Work with interdisciplinary teams to meet organizational goals. Record vital signs and health information in EMR. Analyze patient data to identify needs and treatment goals. Assess physical conditions and support diagnosis and referrals. Explain medical information to patients to educate them. Maintain communication with primary care providers and support transitions of care. Build rapport using motivational interviewing techniques. Conduct extended in-person appointments and follow-up visits. Document encounters accurately and prepare reports as needed. Participate in team meetings, daily huddles, and clinical rounds. Perform other duties as required. Knowledge, Skills & Abilities Strong critical thinking skills for assessing needs and setting treatment goals.
