Evergreen Post Acute is hiring a full-time Registered Dietitian to join the clinical team at our 151-bed skilled nursing and rehab community in Smyrna.

Skilled Nursing & Rehab Facility
Salary: $70,000–$85,000, based on experience

What You'll Do
Complete admission, quarterly and significant-change nutrition assessments for post-acute and long-term residents
Develop individualized care plans.
Participate in interdisciplinary care conferences and weekly risk meetings, where your recommendations shape outcomes
Contribute nutrition sections for MDS and support accurate PDPM capture
Document in PointClickCare
What You Bring
Active RD/RDN credential in good standing
Delaware LDN license, or eligibility to obtain one
Bachelor's degree or higher from an ACEND-accredited program
Preferred, not required: long-term care experience and familiarity with MDS, PDPM and NFPE. If you're coming from acute care or outpatient work, we'll train you on the LTC side.
