Hospice On-Call Registered Nurse (RN) provides after-hours nursing support for hospice patients and families. This role is responsible for responding to urgent patient needs, performing visits when needed, support for symptom management, providing emotional support, and coordinating care during non-business hours. The RN works a 7-days-on / 7-days-off schedule , ensuring continuous coverage for hospice patients requiring immediate clinical assistance.

Patient Care

• Respond promptly to after-hours patient calls, triage requests, and urgent care needs .
• Perform in-home visits when clinically necessary to assess and manage patient symptoms.
• Provide pain and symptom management according to hospice care plans and physician orders.
• Assess patient condition and identify changes requiring intervention.

Clinical Coordination

• Communicate with the hospice interdisciplinary team , including physicians, social workers, and chaplains.
• Implement and update individualized care plans as needed during on-call hours.
• Coordinate with pharmacies and medical equipment providers for urgent needs.

Family & Caregiver Support

• Educate families and caregivers on end-of-life care, symptom management, and medication administration .
• Provide emotional support and guidance during crisis situations or patient decline.
• Assist families during active dying and post-death procedures when necessary.

Documentation

• Complete timely and accurate clinical documentation in the electronic medical record (EMR).
• Record patient assessments, interventions, and communication with the care team.

Compliance & Safety

• Follow all hospice regulations, standards of practice, and organizational policies .
• Ensure care aligns with Medicare hospice guidelines and patient care standards
