Pharmacist - Inpatient

Pharmacist - Inpatient Position Overview: You provide inpatient pharmaceutical care in hospital settings, ensuring safe, effective medication therapy across departments and patient populations. You collaborate with care teams to optimize regimens, manage drug interactions, and support timely transitions of care. Responsibilities Review inpatient orders for safety and accuracy; verify dosing and administration. Collaborate with physicians, nurses, and other pharmacists to optimize therapy and resolve drug‑related problems. Participate in rounds and contribute to clinical decision‑making for complex cases. Monitor for adverse events and implement safety interventions as needed. Provide patient education on medications and discharge instructions. Document interventions, adjustments, and outcomes in the EHR and hospital systems. Qualifications Education: Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy. Licensure: Active pharmacist license to practice in the state; hospital experience preferred. Certifications: Immunization certification Basic Life Support (BLS) certification Experience or Skills: Experience in inpatient hospital pharmacy; strong clinical and communication skills; proficiency with hospital EHRs. Experience in inpatient pharmacy or equivalent setting Experience with medication safety programs and transitions of care Residency or pharmacy specialty training in acute care Experience with antimicrobial stewardship or complex regimens Work Environment Hospital inpatient units with cross‑disciplinary teams; variable shifts and on‑call responsibilities; documentation in the EHR. Fast‑paced environment emphasizing accuracy, safety, and collaboration with clinical teams.
