Summary
Responsible for providing nursing services and education within the scope of practice as set forth by the North Carolina Board of Nursing or South Carolina Board of Nursing to individuals and their families in a homecare or a facility setting. Coordinates plan of care by working with the interdisciplinary group to anticipate and meet patient needs. Serves as the RN member of interdisciplinary group, to meet specific plan of care goals. Coordinates closely with related disciplines to ensure professional service delivery.

Minimum Qualifications
- A Diploma or associate degree in nursing is required; bachelor's degree preferred.
- A current, active, and unrestricted compact (multistate) license as a Licensed Registered Nurse (LPN) or Registered Nurse (RN) is required.
- Current CPR required; CHPN preferred.
- Minimum 2-year related experience. Experience in direct care of patients with life limiting illnesses required. Related experience may include hospice and palliative care; family medicine; internal medicine; geriatrics; oncology; home health care; medical surgical.
- Computer proficiency is required
Knowledge, Skills, and Abilities
- Excellent interpersonal and communication skills (both written and verbal). Strong problem solving and critical thinking skills. Well organized, self-directed, and able to manage time. Flexible, innovative, resourceful, and creative.
- Ability to interact with patients and family members in a varying psychosocial circumstance complicated by the presence of terminal illness. Ability to be diplomatic yet assertive.
- Knowledge of nursing principles, practices, and policies, and sound clinical skills. Understanding of hospice, palliative, and/or home healthcare policies, and procedures. Strong belief in the company mission.
- Excellent ability to work within a team. Excellent ability to coordinate and foster multi-interdisciplinary care.
- Reasoning Ability: Ability to deal with a variety of variables. Able to interpret various instructions.
Essential Functions
- Delivery of Patient Care
- Drive transitions to hospice or perform the hospice admission as appropriate.
- Continuously monitors and reassesses patient and family needs, symptoms, and overall condition, demonstrating a clear understanding of the established plan of care; promptly communicates changes in patient status to the appropriate care team to support timely, informed clinical decision-making and appropriate care adjustments.
- Serves as the liaison for the primary care and/or palliative care providers regarding providers' orders for the care plan; responsible for continued communication with providers and other health care personnel concerning changes in medical treatment and all other aspects of patient care.
- Provides education and counsel to the patient/family regarding palliative care and their plan of care.
- Coordinates community resources for patients' needs as assessed by the palliative care team in conjunction with the social worker.
- Apply infection control principles and follow universal precautions in accordance with accepted professional standards.
- Follows clinical guidelines, protocols, workflows, metric requirements, quality guidance, and regulations specific to VIA Health Partners.
- Will adhere to external agency rules and policies while on site delivering patient care.
- Compassionately recognizes changes in a patient's condition that may indicate hospice eligibility and collaborates with providers, patients, and families to support thoughtful, informed transitions to hospice care that honor the patient's goals, values, and wishes.
- Interdisciplinary Group
- Attends, participates, and may facilitate interdisciplinary group conferences and special care conferences.
- Assumes responsibility for establishing and maintaining effective working relationships with team members.
- Identifies specific patient/caregiver needs and make referral to appropriate team members.
- Assists the Patient Services Coordinator with managing the referral process, and provider assignments.
- Offers guidance, mentorship, and preceptorship to team members, as needed, to support team development.
- Provides cross-coverage for team members, as needed, to support continuity of care and team operations.
- Documentation
- Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and all licensure and certification requirements.
- Reviews and updates pain contracts for each palliative care provider per guidelines.
- Professional and Personal Accountability
- Demonstrates competency in knowledge and skills.
- Timely hospital conversions as deemed by leadership.
- Ensures that VIA Health Partners professional reputation is maintained and projected.
- Demonstrates accountability for continuous professional growth by staying current with evidence-based practice, clinical research, and healthcare trends, and consistently incorporates this knowledge to enhance patient care and clinical decision-making.
- Maintains health requirements, yearly competencies, current professional licensure, CPR certification and CEU requirements as appropriate.
- Demonstrates strong time management skills by effectively planning the workday while remaining flexible and responsive to emergencies or crises.
- Routinely checks weekly scheduled assignments.
May perform other duties as required.
Scope of Responsibility
Demonstrates the ability to practice independently while delivering nursing care in alignment with established professional standards, organizational policies, regulatory requirements, and clinical procedures, with performance oversight focused on charting, quality outcomes, metrics, and overall effectiveness. Responsibilities may include, but are not limited to, serving as both a clinical and relationship management resource between VIA Health Partners and assigned facilities; identifying appropriate patients for transition; facilitating access to palliative care services; educating patients, families, and staff; and supporting transitions to higher levels of care as patient needs evolve.
