JOB TYPE Classification: Exempt JOB SUMMARY QUALIFICATIONS Education: Bachelor's Degree in Nursing or Bachelor's Degree in Social Work accredited by the Council on Social Work Education or Bachelor's Degree in related healthcare field. Experience: Three year’s experience in Case Management or related experience. Leadership experience.Refer to the Life Support Training Policy for additional details. Licensure: Current RN licensure in appropriate state. Certification in Case Mgmt. or related field. Excellent written and verbal communication skills including the ability to write clearly and accurately in non-technical terms about complex policy. Ability to consult on matters related to policy in an effective manner. Demonstrated career progression with increasing roles of responsibility. Commitment to working in a team environment with an emphasis on collaboration. Ability to demonstrate initiative, ownership and accountability of mid-size projects. Ability to function effectively in an environment with multiple and fluctuating priorities; successfully manage multiple high priority projects simultaneously. Acts as a facilitator of discharge planning process improvement projects and actively participates in length of stay and throughput process improvement projects. Responsible to ensure that all staff has valid licensure, other credentials or certifications. Develops and implements plan for self-development in collaboration with the Leaders. Education and training will occur initially during orientation and will be validated per organizational policy. Facilitates length of stay/outlier case review discussions. Monitors customer satisfaction scores and makes changes for improvement. Works with physicians and all members of the Interdisciplinary team (when applicable) to develop or improve processes around the care of their patients. Serves as a patient advocate.Enhances a collaborative relationship to maximize the patient’s and family’s ability to make informed decisions. Human Resources and Organization Development: Provides ongoing and annual feedback to staff on performance consistent with organizational policies and procedures, including performance improvement plans. Coaches staff in developing plan to achieve personal career goals. Provides for education and training of staff. Builds teams and partnerships. Follows the process for open job posting and allows employees equal access to opportunities to develop themselves. Monitors, evaluates and coordinates workload trends and manages resources to meet unit needs. Coordinates planning for human resource needs based on productivity and workload. Continually seeks to implement activities that reduce staff turnover rate. Financial Management: Manages positions within the department to achieve quality and productivity, and meet budget. Provides input into the annual operations and capital budgets. Manages the department budget (measures, monitors, and balances human and material resources). Provides input and collaboration for strategic planning for system/facility and community. Analysis: Analyzes processes and creates deliverables to provide information and recommendations for improvements. Breaks down problems and issues into sub-components and assesses the costs, benefits, and risks of various options. Is able to select and recommend the best solution based on a thorough examination of all considerations. Is able to explain and justify actions in a systematic and logical fashion. Communication: Shares and receives information using clear oral, written, and interpersonal communication skills. Demonstrates effective written and oral communication skills. Actively listens, provides constructive feedback, and demonstrates respect for differing views. Tailors communications to diverse audiences. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
