Your Responsibilities in this Role

• Conducting thorough patient assessments, arranging for diagnostic evaluations, and crafting personalized care strategies
• Diagnosing and managing acute, chronic, or episodic illnesses in conjunction with physicians or independently (as per credentialing)
• Instructing patients and families on health promotion, disease prevention, and effective management of conditions
• Prescribing necessary medications and treatments following the established scope of practice
• Participating actively in patient follow-ups and comprehensive chronic care management
• Cooperating with multidisciplinary teams to ensure cohesive and comprehensive patient care
• Ensuring precise and complete documentation within electronic medical records (EMR)

What You Need to Bring

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Graduation from an accredited Nurse Practitioner program

Necessary Certifications

• Active Nurse Practitioner license in the state of California
• National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or equivalent specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to secure within 90 days)
