Your Role

As a Hospice RN Case Manager, you'll be a trusted clinical presence for your patients and their families. You'll coordinate care, manage symptoms, provide education and support, and collaborate with physicians, caregivers, and the hospice interdisciplinary team.

Responsibilities

• Manage and coordinate individualized hospice patient care plans
• Perform comprehensive nursing assessments and ongoing patient evaluations
• Provide expert symptom management and compassionate nursing care
• Educate patients and families about disease progression, medications, and comfort-focused care
• Collaborate with physicians, caregivers, and interdisciplinary team members
• Maintain accurate, timely, and thorough clinical documentation
• Provide emotional support and guidance to patients and loved ones
• Advocate for patient comfort, dignity, and quality of life

What We're Looking For

Qualifications

• Active Pennsylvania RN license
• Strong clinical assessment and organizational skills
• Excellent communication and interpersonal abilities

• Valid driver's license and reliable transportation
