Requirements
- 1+ years of PT Experience
- Outpatient Experience
- Active Illinois Physical Therapist License
- Active American Heart Association BLS Certification

Roles & Responsibilities
- Plan and execute rehabilitation programs tailored to help patients improve physical function.
- Evaluate patients' mobility, strength, and current levels of function, setting realistic and achievable goals for improvement.
- Develop and implement personalized Plans of Care, incorporating short and long-term objectives and involving patients and caregivers in the process.
- Document patient progress in accordance with facility and medical documentation procedures.
- Maintain open communication with therapy staff and other involved healthcare professionals to coordinate patient care.
- Supervise physical therapy assistants as necessary.

Compliance / Notes
- Certifications and authorizations must comply with third-party payer requirements.
- Maintain professional and technical knowledge through ongoing education and professional development activities.
