Registered Nurse (RN)We are seeking an experienced Registered Nurse to join our team as an inpatient float case manager. This role requires expertise in acute inpatient care and case management, focusing on providing exceptional patient coordination and care throughout the hospital setting.This position offers a 4x10 hour shift schedule, including Saturdays and Sundays. The selection of the other two working days is flexible and open for discussion.As an inpatient float case manager, you will work within the care coordination and case management unit, collaborating with a team of healthcare professionals dedicated to providing high-quality patient care.Client Details State VAJob OverviewPerform case management duties, including patient assessments, treatment planning, and care coordination.Collaborate with interdisciplinary teams to ensure all patient care needs are met effectively.Utilize Epic software for documentation, patient management, and communication within the healthcare team.Advocate for patients within the healthcare system to ensure they receive appropriate services and support.Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.QualificationsCurrent Registered Nurse (RN) license.Minimum of Level 1 acute inpatient experience required.Proficiency in using Epic software for patient management and documentation.Strong critical thinking and problem-solving skills.Excellent communication and interpersonal skills.
