Social Worker

Company Description Your Precious Dreams is an intensive outpatient program in Cleveland, Ohio, serving individuals experiencing addiction, mental health challenges, or behavioral disorders. The organization offers flexible, client-centered care that recognizes recovery as a long-term journey. Programs are tailored to individual needs, whether participants are new to treatment or continuing care. Morning, afternoon, and evening sessions are available several times a week, allowing participants to receive support while maintaining work, school, and family responsibilities. Role Description This is a part‑time, on‑site Social Worker role based in Cleveland, OH. The Social Worker will provide direct clinical support to clients, including completing assessments, developing individualized treatment plans, and facilitating group and individual therapy sessions within the intensive outpatient program. The role includes ongoing case management, coordination with families and external providers, and support with referrals to community resources. The Social Worker will document services in a timely and accurate manner, participate in interdisciplinary team meetings, and contribute to continuous improvement of program services. The position also involves promoting a trauma‑informed, culturally responsive environment and supporting clients in building coping skills and relapse‑prevention strategies. Qualifications Demonstrated skills in clinical assessment, treatment planning, and evidence‑informed counseling or therapy. Experience providing support to individuals with substance use disorders, mental health conditions, or behavioral health needs. Strong communication, active listening, and de‑escalation skills, with the ability to build trusting, professional relationships. Ability to collaborate effectively in a multidisciplinary team and coordinate care with external providers and community resources. Solid organizational and documentation skills, including accurate, timely charting and adherence to confidentiality standards. Commitment to trauma‑informed, person‑centered, and culturally responsive care for diverse populations. Master’s degree in Social Work (MSW) or related behavioral health field preferred; relevant licensure or eligibility for licensure in Ohio (e.g., LSW, LISW) strongly preferred. Previous experience in outpatient, intensive outpatient, or similar behavioral health settings is a plus.
