This Home Health RN Case Manager role is based in Vienna, West Virginia, a welcoming riverfront community in the Mid-Ohio Valley. You will complete comprehensive assessments, develop individualized care plans, manage ongoing follow-up visits, and collaborate closely with physicians and the interdisciplinary team. Typical responsibilities include skilled nursing visits, medication management, wound care, chronic disease monitoring, patient and family education, and timely documentation in the EMR.Your workday will generally involve traveling to multiple patient homes in and around Vienna, managing a caseload that allows for meaningful, relationship-based care. Patient ratios and visit volumes are structured to support thorough assessments and strong patient engagement. You will have the autonomy to plan your daily schedule, prioritize visits based on clinical need, and adjust care plans as conditions change, while knowing you have access to a supportive clinical team and leadership.Shifts are typically daytime hours, with some flexibility to accommodate patient needs and agency scheduling. On-call responsibilities may be rotated among team members, with clear guidelines and support. Registered Nurse, Home Health Experience: 1 year RN(Compact Accepted) SSN Required DOB Required References: 1 Reference in entire work history Driver's License and Auto Insurance
