Hospice Registered Nurse (RN)

JourneyCare Hospice is seeking a dedicated Hospice Registered Nurse (RN) to provide compassionate, patient-centered care in the South Chicago area. This is an excellent opportunity for nurses with hospice, home health, or acute care experience looking to make a meaningful impact.

We are offering competitive pay up to $95,000 annually, a supportive team environment, and strong benefits starting quickly.

Location: South Chicago (Beverly and surrounding areas), IL Schedule: Full-Time | Three 12-hour shifts (10:00 AM 10:30 PM) Includes two weekend shifts per month + one major and one minor holiday annually

Key Responsibilities

Perform hospice admissions and pre-admission assessments
Educate patients and families on hospice care and services
Develop and implement individualized care plans
Provide hands-on nursing care focused on comfort and quality of life
Collaborate with interdisciplinary care teams
Ensure compliance with all clinical and quality standards

Qualifications

Active RN license in Illinois
Graduate of an accredited nursing program
Current CPR certification
Minimum 1 year of hospice experience preferred (home health or med-surg considered)
Strong assessment and critical thinking skills
Ability to work independently in the field
Excellent communication and patient care skills
