The Briarwood Community is seeking a compassionate and knowledgeable Wellness Nurse (RN or LPN) to support Independent Living residents by providing health assessments, guidance, advocacy, and wellness education.

This role serves as a key resource for residents navigating healthcare needs and accessing appropriate medical services while promoting independence, safety, and overall well-being.

Position Type: Part-Time or Full-Time (based on organizational needs)

Department: Resident Services

Schedule:  Monday-Thursday with Alternating Weekends 8am - 4pm

• Conduct resident wellness assessments to help determine appropriate medical care and services.
• Provide guidance and direction regarding healthcare resources, providers, and treatment options.
• Facilitate regular blood pressure clinics for residents.
• Assist residents in navigating healthcare systems, appointments, and medical processes.
• Maintain accurate and timely documentation of resident health concerns and wellness issues.
• Provide professional verbal and written communication with residents, families, healthcare providers, and team members.
• Participate in weekly Risk/Tracking meetings and other interdisciplinary meetings as needed.
• Meet with prospective Independent Living residents and provide wellness assessments during the admission process.
• Collaborate with community agencies, educational institutions, and service providers to develop wellness and educational programs.
• Support the Home Companion Program through assessments, recommendations, and ongoing guidance.
• Advocate for residents while promoting independence and informed decision-making.
• Perform other duties as assigned.

• Current Massachusetts RN or LPN license in good standing.
• Current Basic Life Support (BLS) Certification.
• Minimum of two years of experience in one or more of the following settings:
  • Skilled nursing or nursing center
  • Assisted living
  • Memory care
  • Independent living community
• Experience working directly with residents, families, healthcare providers, and community service organizations.
• Strong assessment, communication, and organizational skills.
• Ability to maintain confidentiality and exercise professional judgment.
• Proficiency with documentation and basic office technology.

This position primarily operates in an office environment while also spending significant time throughout the community interacting with residents and attending resident programs and events.

The role routinely utilizes standard office equipment, including computers, telephones, copiers, printers, and electronic documentation systems.

The Wellness Nurse frequently interacts with residents, family members, healthcare providers, and staff and may occasionally be exposed to infectious diseases, medical conditions, and other healthcare-related risks.

The physical demands described here are representative of those necessary to successfully perform the essential functions of this position.

• Regularly required to communicate effectively through speaking and hearing.
• Frequently required to stand, walk, bend, kneel, stoop, crouch, and occasionally climb.
• May be required to assist residents with mobility and support needs.
• Must be able to lift, carry, and move items weighing up to 50 pounds.
• Ability to move throughout the campus and respond to resident needs in various settings.
