Description Stephens Hospital Clerical/Administrative Support Req #: 64407 Summary As part of the Access to Care team, you work in close communication with our other case managers at Stephens Hospital and across the MaineHealth system to address Social Drivers of Health and provide healthcare navigation and financial resource strain for our patients. Team members utilize a variety of tools, systems, and resources to match patient needs with available resources. The Case Manager - Prescription Assistance role is responsible for interacting with medical providers, pharmacy staff, and staff at pharmaceutical companies to research and resolve recurring and/or urgent medication needs, administer and track the status of Prescription Assistance Program (PAP) applications, advocate on behalf of members, screen program members for eligibility for local/state/federal programs, and assist with finding community resources. Required Minimum Knowledge, Skills, and Abilities (KSAs) Education: Associate or bachelor\'s degree in social services, pharmacy, or healthcare field preferred. License/Certifications: N/A Experience: Two years of experience in social/human services, pharmacy, healthcare management, or related field required. Community outreach experience and networking required. Experience in public speaking required.
