Position Summary This is not a traditional Physician Assistant position. As a Wound Care & Rounding Provider, you'll work with medically complex patients, manage advanced wound cases, and provide ongoing clinical oversight within senior living communities. You'll become a trusted healthcare resource for residents, families, nurses, and community leadership while helping shape outcomes for some of the most vulnerable patient populations in healthcare. Responsibilities Provide on‑site rounding and advanced wound care services across senior living communities. Manage pressure injuries, diabetic ulcers, surgical wounds, and other complex skin integrity concerns. Support chronic disease management and preventative care initiatives. Collaborate with physicians, nurses, therapists, caregivers, and facility leadership. Help reduce avoidable hospitalizations through proactive clinical intervention. Communicate effectively with families, caregivers, and interdisciplinary care teams. Complete documentation, medication management, diagnostics, and lab coordination. Qualifications Active Minnesota Physician Assistant (PA-C) license in good standing. Experience in wound care, geriatrics, primary care, post‑acute care, internal medicine, or related settings preferred. Strong clinical judgment and confidence managing medically complex patient populations. Valid driver's license and reliable transportation. Commitment to high-quality, outcomes‑driven senior care.
