Nurse Case Manager

The Town of Acton Nursing Services is looking for a 28.8 hour per week Nurse Case Manager who utilizes the nursing process, provides comprehensive, coordinated and client-based home health services for individuals and their families reporting to the Clinical Case Manager.
Duties: Provides appropriate, preventive and rehabilitative care through assessment, teaching, counseling and direct care to clients, private caregivers, and/or family members. Coordinates care with other community resources as needed. Coordinates care with other team members and attends case conference and staff meetings weekly. Completes OASIS documentation and daily visit notes in a timely manner based on agency policy and regulatory guidelines. Twice per month on-call weekend requirement. Minimum Entrance Requirements : Registered Nurse licensed in the Commonwealth of Massachusetts. Graduate of an accredited college or university. Valid driver's license.Preferred Qualifications : Two (2) years of medical-surgical or subacute experience. Two (2) years of home health experience. Clinical OASIS certification preferred. Strong medical software experience. Familiarity working with diverse populations, including non-English speakers and sign language communities. Ability to use translation software and digital communication tools to support effective communication. Special Requirements: Must hold current license to practice nursing in Massachusetts. Must be available to work 2 weekends on call per month. Starting Range: $67,004.72 - $75,414.34 annually.
