We offer a comprehensive benefits package and a collaborative atmosphere dedicated to resident recovery and independence.Position OverviewStatus: Full-Time, BenefittedRate: $60-68 DOEExperience: Experienced PTs or New Grads Welcome!Location: San Jose, CARole: Evaluate and treat residents, lead clinical training, and manage individualized rehabilitation plans.Essential Duties & ResponsibilitiesClinical Excellence: Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders.Treatment Planning: Develop and implement individualized plans focused on improving mobility, strength, balance, and functional independence.Supervision & Training: Supervise Physical Therapy Assistants (PTAs) and assist the nursing department with the training of Restorative Aides.Collaboration: Communicate regularly with physicians, nursing staff, and families regarding progress, goals, and discharge planning.Documentation: Complete timely and accurate clinical documentation in compliance with facility, state, and federal requirements.Equipment Management: Recommend and facilitate the ordering of durable medical equipment (DME) such as walkers, canes, and splints to enhance resident independence.Education: Provide in-service training for facility staff and educate families on fall prevention and home exercise programs.QualificationsEducation: Graduate of an accredited Physical Therapy program.Licensure: Current and active California Physical Therapist license (or eligibility for new grads).Skills: Ability to relate positively and effectively with residents, families, and professional colleagues.Integrity: Strong commitment to confidentiality and resident rights.Why Join Us?Full Benefits Package: Medical, Dental, and Vision coverage.Supportive Environment: A team-oriented approach to rehabilitation.Growth: Opportunities for ongoing clinical education and professional development.Stability: Work within a well-established facility committed to quality assurance and performance improvement.We are an equal opportunity employer.
