Our 121-bed skilled nursing facility is located in the heart of Lewistown, Pennsylvania, within walking distance to The Geisinger – Lewistown Hospital. We are seeking a compassionate and skilled LPN (Licensed Practical Nurse) to join our team and help us continue our tradition of providing high-quality care to our patients.

About the Role:

As an LPN (Licensed Practical Nurse) at William Penn Nursing and Rehab, you will play a vital role in delivering exceptional patient care to our residents. Your duties will include:

• Providing direct patient care, including administering medications, wound care, and treatments as prescribed by physicians

• Developing and implementing individualized care plans to meet the unique needs of each patient

• Monitoring patient vital signs, reporting changes to the nursing team, and implementing interventions as needed

• Collaborating with other members of the healthcare team to ensure comprehensive care and coordination

• Participating in shift reports and communicating with colleagues to ensure continuity of care

• Maintaining accurate and up-to-date patient records and documentation

Requirements:

• Current LPN (Licensed Practical Nurse) license in the state of Pennsylvania

• High school diploma or equivalent required
