Hospice Care Consultant
St Croix Hospice
St. Croix Hospice in Bloomington, MN is seeking a Care Transition Coordinator to develop strong relationships with referral sources and designated communities, while educating clients on hospice care. You will assist intake to ensure timely referral coordination. You will meet admission targets, coordinate daily sales and marketing operations, and maintain CRM records. This role supports budgeting, market assessments, and collaboration with the care team to improve patient access and outcomes. #J-18808-Ljbffr
