Occupational Therapist (OT) – Full-Time
Lock Haven Rehabilitation & Senior Center is seeking a compassionate and motivated Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team. This is an excellent opportunity to work in a collaborative Skilled Nursing and Long-Term Care environment dedicated to helping residents maximize their independence and quality of life.
Position Details
- Full-Time
- In-House Rehabilitation Program
- Skilled Nursing Facility (SNF) / Long-Term Care (LTC)
- Collaborative interdisciplinary team
- Consistent caseload
- Supportive leadership and mentorship

Qualifications
- Current Pennsylvania Occupational Therapist license or eligibility for licensure
- Strong clinical, communication, and documentation skills
- Commitment to patient-centered care
- Ability to work effectively within a multidisciplinary team
- SNF/LTC experience preferred, but not required
Technology
- Net Health EMR
- PointClickCare (PCC)
Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide skilled occupational therapy services focused on improving independence with daily living activities
- Collaborate with physicians, nursing staff, and rehabilitation team members
- Complete timely and accurate documentation
- Educate residents, families, and caregivers on treatment goals and progress
- Ensure compliance with facility, state, and federal regulations
