Family Practice - Without OB Physician

Job Function

Join a nation-wide primary care organization focused on providing exceptional care to the geriatric population. We are seeking dedicated professionals to join our collaborative care team.

• Schedule: Monday-Friday, 8am-5pm (no nights or weekends)
• Patient Load: 12-16 patients per day once fully ramped up
• Patient Demographics: Primarily Medicare (over age 65), with some Medicaid patients under age 65
• Collaborative Care Team: Includes 1 MD, APP, Scribe, MA, and shared RN/Social Work support
• Specialty Support: Partnership with a telemedicine company for quick access to specialists
• Technology: EPIC EMR with scribe support and advanced patient care algorithms
• Call Schedule: Rotated amongst market, approximately 1 week every 3 months, 100% remote

Position Details

This is a full-time permanent position offering:

• 4-6 week orientation/training period

Requirements

• Board Certification Required
• CT State License Required
• Training as a Family Medicine, Internal Medicine, or Geriatric Physician
• Previous primary care or skilled nursing facility/nursing home experience preferred, but not required
