Youth Independent Living Transition Coach

Lutheran Metropolitan Ministry in Lakewood, OH is seeking an Independent Living Transition Specialist to provide core independent living skills to youth aged 16–22 in foster care or aging out. The role uses a trauma-informed, wrap-around approach to develop life skills, plan ISP goals, and monitor progress.
