Clinical Admissions Specialist - FT Day - Hoeger House - Post Acute Care

Job Summary Sanford Health is hiring a Staff Resident/Client Admission Coordinator to identify potential admissions, process and place new resident/client requests, and track admission status. The role supports facilities and agencies with admission‑related tasks, collaborates with referral sources, coordinates move‑in process, performs clinical evaluations, and ensures compliance with regulations. The position requires a professional healthcare license and experience in long‑term care. Responsibilities Identify potential admissions and process new resident/client requests Assist facilities/agencies with admission‑related tasks and authorizations Coordinate resident/client move‑in, including paperwork, insurance verifications, prior authorizations, and transportation Perform clinical evaluations to determine appropriate services Determine actions for cases that do not meet facility/agency standards Collaborate with discharge planners, family members, and interdisciplinary team to promote location services Comply with related laws, regulations, policy, and procedures Educate healthcare team on trends, external regulations, and internal policies Enter data accurately into Customer Referral Management (CRM) and the electronic medical record (EMR) Provide high‑quality customer service and act as point of contact for training and assistance Qualifications Registered Nurse (RN), Licensed Practical Nurse (LPN), Occupational Therapist (OT), Physical Therapist (PT), Licensed Social Worker (LSW) with current state license; or Certified OT Assistant (COTA) / Physical Therapist Assistant (PTA) with state license if working in home‑based services Bachelor’s degree preferred but not required Prior experience in long‑term care facility preferred Valid driver’s license and some travel required Basic Life Support (BLS) certification required Ability to handle confidential information, coordinate multiple responsibilities, and deliver professional customer service Benefits Salary range: $29.00 – $46.50 per hour Shift differentials and full‑time scheduling (40 hours per week, day shifts) Health, dental, and vision insurance Health Savings Account (HSA) Company‑matched 401(k) retirement plan EEO Statement Sanford Health is an Equal Employment Opportunity/affirmative action employer.
