Looking for HEDIS Specialists to work in the field who have previous medical records/HEDIS chart collection experience. Must be okay with traveling to different locations: Manhattan, Brooklyn, Queens and Bronx. 1 day a week in Garden City, NY location (corporate office)

• Non-clinical boots on the ground individuals with experience with HEDIS and Risk chart collection and Part D measures (refills, 90-day conversions etc).
• Ability to scan paper charts for collection and load into Reveleer (through gateway or iron keys dropped off to HCP IT)
• Ability to coach office managers on identifying members who need appointments scheduled
• Ability to be trained and speak to provider incentives offered in Q4 push (detail/data and summary sheets would be provided along with the training from HCP).
• Outlines operational workflows, timelines and technical channels; required HEIDIS/chart chase experience. Responsible for retrieving, reviewing, and extracting clinical data from patient medical records;
• Must be able to go on site to Garden City office in Long Island 1x time per week
• Field position that will be supporting up to 92 practices
• These practices are located in Brooklyn, Queens, Long Island, Staten Island, Manhattan
• Must have reliable transport as they will be commuting between multiple practices
• Going to provider practices to retrieve unfiled charts
• Will be responsible digitize paper charts
• Opportunity to convert for the right person
• 3 months to start as a temp position
• Full-time hours per week -  Monday-Friday
• This is not a remote position
