Population Health Subject Matter ExpertAt Advanced Primary Care, we specialize in patients who are 65+ with multiple chronic conditions like diabetes, hypertension, heart failure, and COPD. Join our multidisciplinary team consisting of physicians, APPs, nurses, pharmacists, social workers, and MAs. We collaborate together to deliver innovative and quality care.Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.Serves as a population health subject matter expert in the practice. Plans and prioritizes care for individuals and population of patients, focusing on strategies that will promote optimal health within populations and directs patients to the right care team member at the right time.
