Description

- Assess patients in both inpatient and outpatient settings to develop personalized treatment plans

- Provide physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function

- Collaborate closely with nurses, physicians, case managers, and various rehabilitation professionals to ensure optimal outcomes

- Educate patients and their families on therapeutic objectives, discharge processes, and home exercise routines

- Maintain accurate documentation of patient evaluations, treatments, and progress in line with facility and regulatory requirements

- Participate in multidisciplinary team rounds and conferences to guarantee coordinated patient care

Education

Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

Current state licensure as a Physical Therapist

CPR certification is preferred

Additional specialized certifications (e.g., neurology, geriatrics) are beneficial but not mandatory

Skills

Previous experience is strongly preferred

Capability to treat patients with complex medical and mobility challenges

Excellent communication and collaborative teamwork abilities

Proficiency with digital charting and clinical documentation systems
