Care Coordinator II: Hrly FT

Position Summary Assists all patients through the Firelands Health by acting as a patient advocate; coordinates continuity of patient care with internal providers and external healthcare organizations and facilities; and manages high risk patient care. What You Will Do Assists all patients through the Firelands Health by acting as a patient advocate and navigator. Participates in Care Management team meetings and quality improvement initiatives. Supports patient self-management of disease and behavior modification interventions. Coordinates continuity of patient care with external healthcare organizations and facilities. Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits. Coordinates care for patient populations with complex health needs focusing on improving outcomes, reducing risk, and supporting appropriate utilization of healthcare services. Conducts comprehensive, preventive screenings for patients and/or assists all support staff in daily patient interactions as needed. Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans. Facilitates patient medication management based upon standing orders and protocols. Participates on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Medical Home initiatives. Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures. Labeling, documenting, and tracking all specimens or tests ordered/received following set standards and reporting all test results to the physician. Charting care coordination on day of visit to ensure documented, organized and chronological medical record with the submission of timely, accurate and complete information to the Central Billing Office. Promotes wellness by providing patient education materials and communicating the physicians’ advice and instructions to patients/families. Assisting with scheduling appointments, tests or procedures. Practices Human Resource Responsibility. Demonstrates competence in leadership, communication, professionalism, organizational and analytical skills, and technical/professional skills and knowledge in performance of duties and responsibilities rendered to the customer population served. What You Will Need High school graduate with successful completion of an accredited/licensed school of nursing or Medical Assisting program with State Certification preferred. Expectation of certification of Medical Assisting program within one year of employment. If applicable maintain License/Certification issued by the Ohio Board of Nursing/Association of Medical Assisting, CPR certification, and any other mandatory reporting training. 5 years’ experience in a clinical setting required. 1-3 years’ experience in chronic disease management, case management, utilization management, and adult acute care preferred. Proficient computer skills, including Microsoft Office (specifically Word and Excel). Highly organized and well-developed oral and written communication skills. Must possess a valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request. Successful completion of a 90 day get-acquainted period.
