Licensed Practical Nurse (LPN) - Long Term Care

Licensed Practical Nurse – Geriatrics

Schedule: 3:00 PM - 11:30 PM, 5 days a week, including every other weekend and holiday.

Assignment Details: Full-Time (40 hours per week), contract running into February/March with a flexible end date.

Requirements:

• Current NY LPN Licensure
• BLS certification (AHA or RQI only)
• Minimum of 1 year of experience as an LPN

Responsibilities:

• Collect ongoing data relevant to residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
• Contribute to the creation of culturally relevant care plans that reflect nursing diagnoses and interventions.
• Recommend revisions to care plans based on evaluations of resident outcomes and identify needs for referrals.
• Maintain current knowledge and competency in nursing practice and assist in mentoring/orientation of staff.
• Provide leadership under the direction of the RN within the professional practice setting.
