• 100% of patients have a WellMed primary care provider-ensuring continuity and coordination of care.
• Focused on geriatric patients with Medicare Advantage coverage.
• Low daily census: Typically 13-15 patients per day.
• Emphasis on whole-person care-addressing physical, mental, and social health.
• Bonus incentives tied to quality metrics, not patient volume.
