RN - Hospice & Palliative Care NurseTake charge of developing, implementing, and managing individualized patient care plans in a hospice setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes and provide compassionate end-of-life care. Engage actively in delivering exceptional customer service and fostering respectful relationships with patients, families, and colleagues.Create personalized care plans based on medical and nursing diagnoses for patients ranging from newborns to geriatrics in hospice care.Deliver skilled nursing services aligned with the established care plan, focusing on symptom management and patient comfort.Communicate patient assessments and care updates regularly with hospice staff and volunteers.Work collaboratively with physicians, patients, families, and other care providers to adjust care plans as needed.Supervise Home Health Aide services according to policy guidelines.Maintain timely and accurate documentation in compliance with agency standards.Build and sustain positive relationships with patients, families, healthcare team members, and referral sources.Observe and assess patients' physical, emotional, mental, and spiritual conditions, coordinating with team members appropriately.Adhere strictly to HIPAA regulations to protect patient health information.Proficiency with MS Office and strong keyboarding skills.Effective communication abilities.Strong multitasking and prioritization capabilities.Application of fundamental nursing principles.Integrity: Demonstrate honesty and ethical behavior, inspiring others through leadership.Compassion: Exhibit empathy and high-quality customer service in all interactions.Accountability: Take ownership of projects and adapt flexibly to changing priorities.Respect: Collaborate effectively with diverse populations, maintaining a positive service orientation.Excellence: Communicate and lead with strategic and tactical expertise, facilitating process improvements.Graduate of an accredited nursing program.Several years of clinical nursing experience.Preferred: Bachelor of Science in Nursing (BSN).Preferred: Previous hospice and/or home health experience.Preferred: Certification as a Hospice and Palliative Care Nurse.Current Registered Nurse license in the relevant state.BLS certification required within three months of hire.Valid driver's license and proof of auto liability insurance as required.This role offers competitive compensation, a comprehensive benefits package, and the opportunity to make a meaningful difference in the lives of patients and families in the Midwest region.
