Hospice RN

Role Description This is a full‑time, on‑site Hospice RN role located in the Greater Cleveland area. The Hospice RN provides direct, holistic nursing care to patients with life‑limiting illnesses in homes, facilities, and hospice settings, focusing on comfort, symptom management, and quality of life. Day‑to‑day responsibilities include assessing patient needs, developing and updating individualized care plans, administering medications and treatments, and monitoring pain and symptom control. The RN collaborates closely with physicians, social workers, chaplains, therapists, nursing assistants, and volunteers to coordinate interdisciplinary care and ensure continuity of services. Additional responsibilities include educating patients and families on disease progression and care strategies, documenting assessments and interventions accurately, participating in on‑call rotations as assigned, and supporting families through the emotional and practical aspects of end‑of‑life care. Qualifications Strong foundation in Nursing and Medicine, with the ability to perform comprehensive assessments and implement evidence‑based interventions. Demonstrated experience or training in Palliative Care, including pain and symptom management and holistic, patient‑centered care planning. Experience in Home Care and Direct Patient Care, with the ability to work independently in varied care environments while maintaining safety and quality standards. Current Registered Nurse (RN) license in the state of Ohio, in good standing. BLS/CPR certification required; hospice or palliative care certification preferred. Excellent communication and active listening skills, with the ability to support patients and families from diverse backgrounds with sensitivity and professionalism. Reliable transportation, valid driver’s license, and ability to travel within Medina, Summit, Wayne, and Cuyahoga counties as needed.
