Social Work Care Manager

Intermountain Health

Intermountain Health is seeking a Social Work Care Manager to support patients across care settings in Salt Lake City. In this role, you will complete biopsychosocial assessments, develop care plans, and connect patients with community resources to address medical and social needs. You’ll collaborate closely with an interdisciplinary team, provide crisis intervention and brief counseling, and coordinate safe care transitions.
