Complex Care Clinical Social Worker

Job Summary The Social Worker is responsible for performing social work assessments and interventions for inpatients, outpatients, and emergency department patients. This role includes comprehensive assessments and the development of treatment plans that integrate medical, social, and resource issues impacting patients, families, and at‑risk populations. Essential Functions Provides psychosocial assessments of patients and families with social, emotional, interpersonal, or environmental issues. Formulates biopsychosocial assessments, dispositions, and treatment plans. Collaborates with other professionals to evaluate patients’ medical or physical condition and assess client needs as part of the interdisciplinary team. Monitors, evaluates, and records client progress according to measurable goals described in the treatment and care plan. Qualifications Master's Degree in Mental Health & Behavioral Medicine or Master’s Degree in Social Work required. Driver’s License required. Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required. 2+ years of clinical experience in a medical setting required. Case management experience preferred. Additional Knowledge, Skills, and Abilities Demonstrates proficient clinical judgment and application of professional values and ethics. Forms risk assessments, crisis interventions, and psychosocial assessments in a timely manner. Advocates for clients while maintaining professional boundaries. Proficient in internal and external resource referral. Provides evidence‑based interventions. Coordinates and communicates treatment plans with patients, families, and the interdisciplinary team. Utilizes supervision and consultation when appropriate. Seeks assistance with complex cases in a timely manner. Engages in quality improvement projects and uses data to measure progress. Facilitates skill‑based groups. Negotiates and works effectively within a complex organization. Communicates findings to a broader audience. Incorporates social sensitivity. Additional Job Details Schedule and Work Model: Full‑time, Monday through Friday, approximately 8 AM to 4:30 PM. Hybrid – approximately two days in office at Harbor Medical Practices, two days in the Pembroke/S. Weymouth communities, and one day at home. On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Video is required and must be accessed using MGB‑provided equipment. Pay Range: $67,537.60 – $98,238.40 per year. Grade: 7. Regular Work Shift: Day.
