About the Position

Neuro/Tele experience preferred. License: Active, unrestricted Registered Nurse (RN) license in the applicable state. Certifications: BLS required. Description: The RN implements professional nursing practice centered on the patient as a whole person and the creation of a caring environment focused on healing and wellness. Nursing practices are based on evidence and current research as well as policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. The RN coordinates, ensures, and provides direct patient care activities on the unit through: provision of nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is a part of the RN's role. The RN provides patient and family-centered education, care coordination, and individualized treatment planning as applicable. The RN performs and documents a comprehensive assessment in accordance with scope of practice and policies/procedures. The RN considers the patient’s culture, age, and developmental level. Identifying patient risk factors and presenting complaints is crucial. The RN collects objective and subjective data while verifying the initial admission assessment if obtained by other members of the healthcare team. A problem list is developed, prioritizing patient needs based on input from the inter-professional team as appropriate. Actions and interventions relevant to the patient’s culture and age are designed. Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care. The RN fully implements patient care therapies and procedures with empowerment and clinical autonomy. Procedures, treatments, and processes are explained to patients and families. The RN initiates action to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessment and response to procedures and therapies are conducted and documented using the nursing process. Tasks are delegated appropriately, and team members' performance regarding patient care is overseen. The RN monitors the accuracy and completeness of delegated patient care provided by team members in the implementation of the plan of care. Educational needs of patients related to prioritized problems according to their level of comprehension are addressed. The RN facilitates the discharge process by executing discharge orders in a timely manner, identifying potential discharges, and coordinating with Case Management and others as indicated. Input into patient/nurse assignments is provided in consideration of available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is demonstrated. The RN promotes a positive group dynamic and healing environment through words and actions. Nursing excellence is assured in accordance with accepted standards, evidence-based practice, current research, and a caring philosophy. Participation in department and/or unit quality improvement activities and core measures is essential. A holistic perspective regarding patient care is utilized.
