Nurse Practitioner (Intensivist)

Summary This position is eligible for the Education Debt Reduction Program—a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval determines award amount (up to $200,000) and eligibility period (one to five years) after review of the EDRP application. Former EDRP participants are ineligible to apply. Duties Independently manage a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develop and implement an individualized plan to achieve optimal population health and disease management. Demonstrate leadership and work collaboratively with the interdisciplinary team. Order, interpret labs, tests, and diagnostic studies. Prescribe pharmacologic and non-pharmacologic interventions. Initiate referrals and consults to specialty services. Provide education to patients and their families. Assess and adjust the plan of care as needed to achieve optimal patient outcomes. Exemplify the ICARE core values of integrity, commitment, advocacy, respect, and excellence. Obtain and document relevant health and medical history in the Computerized Patient Record System (CPRS). Perform a physical examination based on history. Identify medical and health risks and needs of the patient/family based on data collection. Formulate an appropriate differential diagnosis from history, physical examination and clinical findings. Establish a plan of care to achieve identified medical and health goals. Prescribe medications, including controlled substances as permitted by licensure. Identify patient education needs and provide relevant education. Initiate referrals and/or consultations as appropriate to achieve an interdisciplinary plan of care, including community agency referrals. Determine effectiveness of the plan through documentation of patient care outcomes. Be proactive in identifying potential emergency/crisis situations and provide appropriate interventions. Perform procedures within the approved scope of practice: diagnostic and stabilization procedures per ACLS protocol, including airway management, needle thoracostomy, cricothyrotomy. Place and maintain peripherally inserted central catheters (PICC), including de-clotting. Place and maintain enteral feeding catheters. Place, maintain, and manage internal jugular, subclavian, and femoral central lines. Place and maintain percutaneous arterial lines. Place and manage Swan Ganz catheters. Place pleural pigtail or chest tubes. Rewire central line catheters (Cordis to triple lumen). Remove chest tubes (pleural and mediastinal), epicardial pacer wires, femoral arterial sheaths, IABP catheter sheaths.
