Our Hospice Aide (CNA/LNA) Will:

• Work independently and take initiative to manage a caseload.
• Have autonomy and flexibility in managing their schedule.
• Utilize varying skills as a member of the clinical team as well as the community relations team.
• Develop meaningfuleffective andprofessionalrelationships with communities families and patients.
• Provide patients assistance with personal care including but not limited to baths skin care back rubs oral hygiene hair care dressing meal assistance and light housekeeping.
• Assist in supporting the patients psychosocial wellbeing through companionship.
• Assess and report on patients condition and significant changes to the RN Care Manager.
• Assist with community relations events and activities including Vitals Clinics seasonal activities education sessions meet and greets etc.

Our Hospice Aide (CNA/LNA) Has:

• An active unencumbered state-required CNA certification.
• 1 years of experience working as a CNA in home health or hospice.
• The ability to work with little direct supervision.
• The ability to interact professionally with patients families and fellow team members.
• An active Drivers License with a reliable vehicle insurance and an acceptable driving record.
• The ability to assist residents with limited mobility lift/push/pull up to 75 lbs.
• The ability to provide a calm manner when in a patients home.
