IMPORTANT: All applications MUST be completed in its entirety to be considered.*NOTE: COVID -19 Vaccinations are recommendedPosition DescriptionThis position reports to Director of Care Management and will function as an essential team member for Honor Community Health Home projects. The Community Health Worker (CHW) works as an essential part of an interdisciplinary team to deliver integrated, person-centered, and comprehensive care to eligible beneficiaries to successfully address the complexity of comorbid physical and behavioral health conditions. This position will work directly with patients and their representatives to assist in the delivery of six core services: comprehensive care management, care coordination, health promotion, comprehensive transitional care, individual and family support, and referral to community and social support services. This position requires tracking and follow up, ensuring details of care are documented in the Electronic Health Record and population health management tool.
