In-Home Physician Per Diem - Danville-Martinsville, VA

Overview Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In‑Home Health Evaluations in the comfort of patients’ homes. This role involves engaging with individuals to review medication and medical history, perform a physical evaluation, and, when ordered by the health plan, administer diagnostic tests such as diabetic eye exams or spirometry. No prescribing, ordering labs, or altering treatment regimens is required. Responsibilities Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes. Help bring flexible, comprehensive and personalized health evaluations right to people’s front doors. Conduct in‑home visits that last approximately 45 minutes, providing medication review, medical history, physical assessment, and any ordered diagnostic tests. Collaborate with the Signify Health team to fulfill the specified role’s requirements. Travel locally and within licensed states as needed and conduct virtual visits when required. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry, diabetic eye exam, etc. Qualifications Active, unrestricted license(s) in the coverage area(s). Multi‑state licenses preferred. Ability to cover primary locations: Danville‑Martinsville. Employment Details Pay: Hourly base rate of $100.00 - $100.00. Anticipated weekly hours: 12.
