Clinical Lead NP: Chronic Care & Telehealth Innovator

Strive Health

Strive Health is seeking a Clinical Lead Nurse Practitioner to guide interdisciplinary care for patients with CHF, CKD/ESKD, and other chronic conditions. You’ll provide longitudinal, home- and telehealth-based care, supported by analytics and collaboration across teams.

The ideal candidate has 2+ years as an NP in cardiology or nephrology, EHR experience, and active licensure. The role includes a 112k–140k annual salary with a 10% bonus potential and hybrid-remote work flexibility.
