Nurse Practitioner

Longevity Health is seeking a Nurse Practitioner or Physician Assistant to manage a panel of long-term care members. You will conduct comprehensive assessments, develop individualized care plans, and participate in facility care team meetings while communicating with families to align on goals of care.

You will support ISNP Model of Care activities, advance care planning, and coordinate with PCPs and other staff to ensure high-quality, patient-centered care within nursing facilities.
