Contract | 13-Week Assignment | Days | Monday – Friday, 8:00 AM – 5:00 PM | Onsite

Key Responsibilities

• Provide occupational therapy services to patients in a home health setting across an assigned territory
• Conduct evaluations and develop individualized treatment plans to support patient functional goals
• Deliver skilled OT interventions focused on improving daily living activities, safety, and independence
• Document patient progress and clinical notes using HCHB (HomeCare HomeBase) EMR
• Coordinate care with interdisciplinary team members to ensure continuity of patient care

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Washington
• Minimum of 12 months of Home Health experience within the past 3 years
• Proficiency or ability to work within HCHB (HomeCare HomeBase) EMR
• Valid driver's license and reliable transportation for home visits within assigned territory

Preferred Experience / Skills

• Familiarity with Spokane County and surrounding areas
• Experience managing a caseload across a multi-county territory
• Strong time management and independent practice skills in a field-based setting
