Position Summary This professional position involves providing intake informational support to individuals with special needs resulting from a developmental disability. The Intake Coordinator maintains intake caseload to determine eligibility for APD services, consults with families, explains the APD eligibility requirement, and manages the Pre‑Enrollment application process. The Work You Will Do Complete the initial screening form, explain the intake process by phone, email, mail, or in‑person, and ensure information is posted and/or emailed to the area supervisor/lead. Process assigned APD application packets, mail them after completing the screening form, and document the application process within the system tracker. Conduct quality review of documentation, obtaining applicant records such as birth certificates, social security cards, Medicaid numbers, and collateral information submitted to the APD Region Office to determine eligibility for APD services. Maintain progress notes, generate Notice of APD Eligibility/Ineligibility in the iConnect system, and initiate the short‑version support plan after eligibility approval. Complete and sign support plan with the client or legal guardian, and the Home Community Based Eligibility Worksheet. Input, update, and accurately maintain referral and applicant information in the iConnect and other electronic systems used by APD. Maintain and review high‑risk case files for individuals aged 3‑5 to ensure continuity of services when the client turns five years old. Perform other related duties or assignments as directed. Minimum Qualifications Valid Florida Driver License or other reliable transportation for work purposes. Ability to travel for work purposes. Experience working with developmental disabilities or mental illness, or allied health/rehabilitative sciences principles and practices. Preference for fluent bilingual–Creole, Spanish speaking and writing. Knowledge, Skills, and Abilities Travel to surrounding areas, group, foster, and family homes. Plan, organize, and coordinate work assignments. Interview applicants for public benefits assistance and services. Work under pressure, multi‑task, and independently. Establish and maintain effective working relationships with consumers, families, providers, and staff across regions. Prioritize and schedule work efficiently and ensure completion of assignments. Use Microsoft Office, web‑based programs, and other electronic systems. Benefits Annual and sick leave benefits. Nine paid holidays and one personal holiday each year.
