Summary:

This role involves obtaining and updating medication histories for patients admitted or pending admission, ensuring accurate medication reconciliation within the electronic medical record system. The specialist prioritizes medication histories and gathers comprehensive medication information from various sources including patient interviews, healthcare providers, and pharmacies.

Key Responsibilities:

• Navigate electronic medical records to complete medication histories for admitted or pending patients.
• Compile detailed and accurate medication lists including drug name, dose, route, frequency, PRN indications, and last dose taken.
• Document medication histories and allergy information promptly and accurately.
• Refer unresolved medication issues to pharmacists and admitting providers.
• Maintain patient confidentiality and secure sensitive information at all times.
• Report safety hazards to supervisors in a timely manner.
• Conduct educational sessions for pharmacy technicians, nurses, and other healthcare staff, including patient education.
• Precept and train pharmacy technicians, students, pharmacists, and nursing staff on medication history processes.
• Assist with data collection for quality improvement initiatives.
• Perform other duties as assigned.

Education and Formal Training:

• Pharmacy Technician Certification Board (PTCB) certification required unless currently enrolled in a PharmD program.
• Registration with the state Board of Pharmacy required.

Experience:

• 6 months to 1 year of experience as a pharmacy technician required.
• Preferred: 2 years of retail pharmacy experience.

Skills and Abilities:

• Strong attention to detail and technical proficiency.
• Knowledge of common medication doses and forms preferred.
• Excellent interpersonal and communication skills to interact effectively with patients and healthcare professionals.
• Ability to prioritize tasks in a high-volume environment and adapt to changing workloads.

Physical Requirements:

• Ability to work independently and manage stressful situations.
• Capability to read physician notes, medication labels, and lists accurately.
• Manual dexterity for operating pharmacy equipment such as computers, phones, fax machines, and automated dispensing cabinets.
• Ability to stand and walk for extended periods (6-10 hours) and lift moderate loads (up to 10 lbs), including stooping and kneeling.
• Effective verbal and written communication skills are essential.
