RN Case Manager - Home Health

About This Role Join Guthrie Home Health as an RN Case Manager in Waverly, where you will provide meaningful, one-on-one care to patients in their homes. This role is ideal for RNs who enjoy independence, patient education, and care coordination. Highlights Pay: $83,200 - $112,569.60 per year Location: Waverly, NY Sign-On Bonus: Up to $25,000 for eligible experienced RNs Schedule: Full-time, Primarily weekday, daytime What You'll Do Manage a home health caseload and coordinate services with a multidisciplinary team. Plan, implement, manage, and evaluate home health services to meet patient needs. Provide nursing care in the patient’s environment. Demonstrate effective time management and flexibility in patient care. Facilitate communication between team members and patients. Supervise LPNs and ensure yearly competency requirements are met. Accept on-call assignments and travel as required. Requirements Education: Graduation from an approved School of Nursing; Bachelor’s Degree in Nursing preferred. Experience: One year of previous nursing experience in Medical/Surgical Nursing required. Prior Public Health/Community Health experience preferred. Licenses & Certifications: Current RN License in New York. Benefits Medical and dental coverage beginning the first of the month following hire. PTO accrual beginning on Day 1.
