RN / Registered Nurse Case Manager - Hospice Job Locations US-AL-DECATUR | US-AL-MOULTON | US-AL-HILLSBORO | US-AL-TRINITY Company Hospice of North Alabama Position Type: Full-Time Overview Office Location: Cullman, AL Coverage area: Morgan County and areas of Lawrence County (Moulton, Hillsboro, Trinity) Schedule: Monday thru Friday, 8am-5pm Are you a Registered Nurse looking for a new opportunity? Hospice of North Alabama is seeking a passionate, dedicated Hospice RN to join our team in Decatur, AL. Our hospice RNs provide expert, patient-centered care at a critical time in people's lives. If you're ready to work in a supportive, fulling environment where your skills and empathy truly shine, As a RN Case Manager You will: Assess / monitor physical, emotional, and psychological needs of patients Create hospice care plans that align with the patients wishes and goals Direct nursing care: administering medications, treatments, and interventions Provide pain and symptom management Educate and support the patient's family and caregivers Collaborate with interdisciplinary team Maintain accurate and timely documentation Participate in on-call rotation as required by local branch Benefits and Perks for You!
