MSW Social Worker

As a key member of our interdisciplinary care team, you will play a vital role in assessing patient needs, developing personalized care plans, and providing emotional support to patients and their families.
Responsibilities:
• Conduct thorough social work assessments to identify patient needs and develop effective care plans
• Collaborate with interdisciplinary teams to provide comprehensive care and support to patients and their families
• Provide emotional support, counseling, and education to patients and their families
• Coordinate and facilitate patient-centered care transitions, including discharge planning and follow-up care
• Develop and maintain relationships with patients, families, and community resources to ensure continuity of care
• Document all patient interactions, assessments, and care plans accurately and timely
