As a Master’s Social Worker (MSW) in Hospice , you’ll play an essential role in helping patients and families navigate the emotional, social, practical, and financial challenges that can accompany a terminal illness. Support Patients & Families Assess the psychosocial needs of patients and families related to terminal illness, end-of-life care, and the home environment Evaluate family dynamics, caregiver capabilities, communication patterns, coping needs, and potential psychosocial risks Provide counseling, emotional support, and crisis intervention to patients and caregivers as needed Help patients and families navigate stress, anticipatory grief, changing family roles, and end-of-life concerns Recognize and support cultural needs, traditions, values, and preferences that may influence care Guide End-of-Life Planning & Resources Educate and support patients and families with advance directives and healthcare decision-making Assist with practical needs including financial, legal, funeral, and end-of-life planning Connect patients and families with community resources and support services Serve as a liaison between patients, caregivers, community agencies, and other available resources Build and maintain relationships with public and private community organizations that can support patient and family needs Collaborate with the Hospice Care Team Participate in developing and updating individualized plans of care with the patient, family, and interdisciplinary team Participate in regularly scheduled interdisciplinary group meetings and contribute psychosocial insight to care planning Partner with physicians, nurses, chaplains, aides, and other team members to provide holistic end-of-life care Help the care team understand significant social, emotional, cultural, and family factors impacting the patient’s experience Advocate for patient and family needs, preferences, dignity, and quality of life throughout the hospice journey Document & Communicate Maintain accurate and timely clinical documentation, assessments, and progress notes Communicate changes in psychosocial needs, risks, and concerns to the appropriate interdisciplinary team members Maintain strong communication with patients, caregivers, community partners, and the hospice team Participate in quality assessment and performance improvement activities Uphold professional, ethical, and regulatory standards for hospice social work What Sets You Apart Deep empathy and emotional intelligence Comfort supporting patients and families through end-of-life experiences Ability to navigate complex family dynamics and sensitive conversations with compassion Strong crisis intervention, counseling, and problem-solving skills Respect for diverse cultural beliefs, traditions, and approaches to death and dying Resourceful, calm, and solutions-oriented approach Excellent communication and organizational skills Genuine passion for hospice and patient- and family-centered care What You Bring Master’s degree in Social Work (MSW) Meets applicable hospice personnel qualifications and state requirements 1+ year of MSW experience in a hospice setting Understanding of hospice philosophy and issues related to death, dying, grief, and end-of-life care Ability to communicate tactfully and compassionately with patients, families, caregivers, community partners, and the care team Valid driver’s license, reliable transportation, and automobile insurance if making home visits Who We Are
